There are any types of notary collective forms ready for notaries. The contents of these forms differ, depending on the state they originate from. This form also renews an address change in relationship with the name change. But, an address change form can be used for changing the address only.

A copy of the certificate of appointment request form is used to regain a duplicate copy of the notary collective certificate of appointment. This form goes to the Secretary of State with the required nonrefundable statutory fee.

Since notary collective forms are legal forms, one should be wholly cautious before obtaining and completing one. A hastily superior or wrongly formatted form can land you in trouble. It must be verified that the form pertains to the state you are complicated in.
